The MHRA has been unambiguous: Mounjaro is a Prescription-Only Medicine, and selling it without a valid prescription is unlawful. That legal framework exists for patient safety, not administrative convenience. Tirzepatide affects heart rate, gastric motility, kidney function under certain conditions, and several medicines taken alongside it. A prescriber checking your history before authorising treatment is not a formality, it is the safety step.
The MHRA has also warned publicly about counterfeit weight-loss pens circulating online and through social media. In 2025 alone, enforcement actions resulted in the seizure of around 20 million doses of illegally traded weight-loss medicines (with an estimated street value of roughly £45 million), and the first UK site manufacturing fake tirzepatide pens was raided in Northampton. Some counterfeit pens seized in earlier operations were found to contain insulin, a serious risk to anyone without diabetes. The MHRA's Yellow Card scheme allows you to report a suspect seller as well as a side effect, and it is worth knowing that route exists.
Red flags when searching online: sellers offering these medicines without asking for a prescription, prices dramatically below the market range, discount codes from unrecognised sources, or no traceable UK pharmacy registration. Any of those should stop you.
Every pharmacy operating legally in the UK, including online pharmacies, must be registered with the General Pharmaceutical Council. Checking takes under a minute. Visit the GPhC pharmacy register, search by name or registration number, and confirm the premises are listed as active. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI of 27 or above alongside at least one weight-related condition such as high blood pressure, high cholesterol, type 2 diabetes, prediabetes or obstructive sleep apnoea. Lower BMI thresholds can apply for some ethnic backgrounds under UK clinical guidance. NICE recommends tirzepatide for NHS use under specific criteria set out in NICE technology appraisal TA1026; NHS availability is being phased in gradually, with eligibility thresholds tightening considerably compared with private licensed use.
