Does Wegovy Come With Needles — and What Else Is Inside the Pen?

What the pen contains, and the misconception worth clearing up

A lot of people assume they'll need to order needles separately, the way some older injectable medicines work. With Wegovy, that's not the case, because the pen comes with needles already included in the box. Novo Nordisk supplies the pen with needles included in the box, typically a small number of pen needles alongside the pen device itself. You attach a fresh needle before each injection, inject, then remove and dispose of the needle straightaway. The pen itself is not reused indefinitely; once its doses are spent, the whole device goes into your sharps bin too.

The pen needles supplied are fine-gauge and short, designed for subcutaneous injection, meaning they go into the fatty tissue just beneath the skin rather than into muscle. Most people find the actual injection far less daunting than they expected. Needle length and injection technique: what you actually decide

The needles included with Wegovy are short, typically around 4mm or 8mm, depending on the pen configuration in your supply. For most adults injecting subcutaneously, a 4mm needle is sufficient regardless of body size; the fatty layer under the skin is thinner than most people imagine, and a longer needle risks going too deep. The decision you genuinely make each week is where to inject. Wegovy can be given in the abdomen, the outer thigh, or the upper arm. Rotating the site (not just the location but the spot within that location) lets the tissue recover between doses. Some people settle into a comfortable routine quickly; others find it useful to keep a simple note of where they injected last. Neither approach is wrong.

If you are using alcohol swabs to clean the site beforehand, medical-grade swabs are the right choice, let the skin dry for a few seconds before injecting so the alcohol doesn't sting.

Sharps disposal, a step that matters more than people think

Once the needle is removed from the pen, it needs to go directly into a sharps container. This is not optional. Used needles are a biohazard and cannot go into household recycling or a standard bin, not for safety theatre, but because loose needles cause injuries to bin workers and contaminate recycling streams. A 1-litre sharps container is compact enough to keep in a bathroom cabinet and holds several months' worth of weekly injections.

Most local councils in England, Scotland and Wales run free sharps collection schemes: you drop off a sealed container at a participating pharmacy or council site and collect an empty replacement. The pen body itself (once empty) goes into your sharps container too, not into general waste. If you are travelling with Wegovy, check the entry rules for sharps in your destination country; most permit personal medical sharps in hand luggage with a letter from your prescriber, but rules vary.

When the needles included are not quite right, and what to do

Occasionally a patient finds the needle length supplied does not suit them, or they are going through more injections than expected due to a priming issue. Priming, where a small amount of liquid is expelled before each injection to check the needle is clear, is described in the instructions and is normal, it is not wasting your dose if done correctly.
